Setelah mengalami berbagai peristiwa yang membuatnya nyaris jiwanya melayang, tidak heran jika seorang mantan anggota pasukan elit Green Berets AD AS akhirnya merasa bosan dan ingin hidup tenang tanpa mengenal kata peperangan. Itulah awal kisah Rambo IV, film aksi laga produksi 2008 hasil besutan Sylvester Stallone yang juga menjadi pemeran utamanya.
Dalam film keempat dari saga Rambo ini, veteran perang Vietnam John Rambo (Sylvester Stallone) hidup tenang di Thailand dimana ia sehari-harinya menangkap ular untuk dijual sebagai hewan pertarungan dan mengantar penumpang di sungai dengan perahu motor tuanya. Sayangnya ketenangan hidupnya terusik dengan kedatangan sebuah rombongan misionaris Kristen pimpinan Michael Burnett (Paul Schulze) dan Sarah Miller (Julie Benz).
Rupanya rombongan tersebut ingin melakukan perjalanan ke Burma (Myanmar) padahal negeri itu merupakan zona perang khususnya untuk suku Karen yang menderita di bawah tekanan rezim militer Burma yang brutal. Mereka pun diantar ke sana dan ditinggalkan Rambo sesuai perjanjian. Namun setelah Rambo pulang, para misionaris kemudian ditangkap oleh pasukan Burma yang dipimpin Mayor Pa Tee Tint (Maung Maung Khin) yang sadis.
Pimpinan dari organisasi misionaris mendengar kabar buruk itu. Tidak bisa melihat jalan lain sehingga iapun terpaksa menyewa sekelompok tentara bayaran. Namun hanya Rambo yang tahu lokasi sehingga iapun diminta sebagai penunjuk jalan para tentara bayaran itu. Masalahnya Rambo agak ragu memenuhi permintaan tersebut lantaran tidak ingin kembali terlibat dalam kekerasan dan perang.
